Adopt-a-Driveway Volunteer Sign Up

After completing your application the City will reach out with next steps and further information.

PLEASE NOTE, INDIVIDUALS UNDER THE AGE OF 18 WILL NEED A PARENT OR GUARDIAN TO CO-SIGN THE ACKNOWLEDGEMENT AT THE END OF THIS FORM.

Volunteer Acknowledgement of Risk/Informed Consent

I acknowledge that I will be performing volunteer snow shoveling as part of the Adopt-a-Driveway Program.

As the volunteer, I confirm the following:

• After a minimum of 3 inches of snow has fallen, volunteers have 24 hours from when it stops snowing to:
  • Shovel the sidewalk.
  • Shovel a path from the sidewalk to the main entrance of the house.
  • Shovel the driveway. 
  • Shovel the end of the driveway (otherwise known as the windrow)
• If I arrive at the address and the sidewalk has been shoveled on three consecutive snow events, I understand that the resident will then be removed from the program. I will undertake to advise the coordinator.
• I have the necessary knowledge, experience, and ability to perform these activities/duties competently and safely.
• I acknowledge and agree that as a volunteer for the City of Sarnia, I will perform all of my activities/duties under the direction and control of the City of Sarnia.
• I acknowledge that during my volunteer activities, I am not considered to be an employee of the City of Sarnia; as such, I recognize that I am not covered by the City’s Workplace Safety and Insurance Act (WSIB) in the event of accidental injury or disease.
• I acknowledge that the City of Sarnia does not carry medical insurance for volunteers and that any medical expenses incurred by me due to an accident would be my responsibility.
• I am aware of and accept the responsibilities, obligations and risks that may arise from my performing or participating in these activities.
• I fully understand the City of Sarnia is relying on my statements in this document being true and accurate, and I would not be permitted to participate in the above activities if my statements were inaccurate or untrue.
• At the conclusion of the program, I will return all equipment to the City of Sarnia.
